This form helps us understand what kind of support you’re looking for, and whether we’re well placed to support you. Our goal is to make sure you receive care that is safe, appropriate, and genuinely helpful.

The form usually takes around 5–10 minutes to complete. You’re welcome to share as much or as little information as feels right for you.

Once submitted, we’ll review your responses and be in touch via email. Please keep an eye on your inbox, including your junk or spam folder.

If we believe another service may be a better fit for your needs, we’ll let you know and suggest more suitable options where possible.
